Annual Transportation Provider Compliance Attestation
Every year, to ensure an effective compliance program, Modivcare is required by both federal and state regulators and by our health plan clients to communicate to and monitor specific compliance requirements of our Transportation Providers. To satisfy these requirements, Modivcare distributes training materials to its Transportation Providers to be completed upon contracting and annually thereafter.
We are asking all Transportation Provider owners and drivers that are providing services for Modivcare to review the materials listed below within 30 days of receiving this request.

- Accident/Incident Report Form
- Billing and Claims Tips
- For Out of Network Providers Only: Driver Credentialing Reminder
Once completed, please sign and return the Transportation Provider Compliance Attestation and Transportation Provider Training Roster forms. The documents must be signed by an individual with the authority to attest to the accuracy and completeness of the information provided.
For newly contracted Providers: Please return the Attestation and Roster attached with your contract package.
For Providers completing Annual Training: You should have received a copy of the Attestation and Training Roster via email, a link to both documents is also provided here. Please return your completed Attestation and Roster within 30 days by email attachment to TPcompliance@modivcare.com. You can also mail your completed documents to the Compliance Department at Modivcare, Attn: Compliance, 6900 E. Layton Ave., Suite 1200, Denver, CO., 80237.
