Come July 1, 2016, Nevada Medicaid’s non-emergency transportation services will change from Modivcare to MTM. Please call MTM’s toll-free number, 1-844-879-7341 starting 6/15/2016, to make July reservations.

Member FAQs

  • How do I arrange my transportation?

    It's easy! You are encouraged to call at least 5 business days in advance before your appointment. If reservations are not made with 5 business day notice we cannot guarantee transportation, but we will make every effort to arrange your ride. The Reservation phone number is toll free - 1-888-737-0833. Confirmed urgent appointments may be booked 24 hours a day, 7 days a week.

  • How do I cancel my ride?

    If you need to cancel your trip, please call as soon as possible. We appreciate at least a one- day notice.  You may cancel 24 hours a day, 7 days a week by calling 1-888-737-0829

  • How do I qualify?

    If you currently have full Medicaid you may qualify for transportation assistance. You must have no other way to get to your medical appointments. Your appointments must be for a Medicaid covered service.

  • What if I have a car & can drive?

    Please check the Member Services Website to enter new reservations.

  • What if my ride is late?

    If transportation is running more than 30 minutes late and you have not heard from your driver, simply call Modivcare's toll-free "Where's My Ride?" number 1-888-737-0829 (Please do not use this number for trip reservations).

  • What information will I need to give?

    To book your trip we will need:

    • Your name, address (including apt. number) and phone number.
    • Your Medicaid number.
    • The Doctor/Clinic's full name, address (including office or suite number) and phone number.
    • The time of your appointment.
  • What kind of ride will I get?

    You will receive the level of transportation that is most appropriate for you based on your medical condition. If you live within 3/4 a mile of a bus line, you may be eligible for bus passes. You may be eligible for other transportation; our customer service reps can give you more information.
