Gas mileage reimbursement can be submitted through the Modivcare App or by using a paper trip log available at www.MyModivcare.com. Members can also call Modivcare at (800) 292-7114 for help. The medical appointment must be verified before reimbursement can be issued.

Gas mileage reimbursement requests may be submitted up to 30 days after the trip. Call Modivcare at (800) 292-7114 if you need help with the process.

Members should call Modivcare as soon as they can, or at least one day before the ride, at (800) 292-7114.

  • Say “Will Call” in the automated phone system
  • Use the Modivcare mobile app or text message link, if available Once the will-call is activated, Modivcare will begin coordinating the return ride.
