Per Section VIII(B) of Modivcare’s contract with the Mississippi Division of Medicaid (DOM) dated January 18, 2024, the following liquidated damages have been approved: 

Amount: $1,100 

Approval Date: January 2025 

Reason: DOM did not receive an updated organizational chart within the requisite timeframe to notify them of a staffing change.  

Amount: $6,900 

Approval Date: February 2025 

Reason: Instances in which eligible trips were not fulfilled due to insufficient provider resources. 

Amount: $13,900 

Approval Date: March 2025 

Reason: Instances in which eligible trips were not fulfilled due to insufficient provider resources. 

Amount: $19,300 

Approval Date: April 2026 

Reason: Instances in which eligible trips were not fulfilled due to insufficient provider resources.  

Thess notices are posted to satisfy the contract’s public posting requirement for liquidated damages.
